I shared practical feedback with the team and adjusted my process quickly while protecting patient safety and continuity of care. What clinical and operational indicators would you use to evaluate a new patient-care program? I would monitor patient access, follow-up completion, medication and referral adherence, avoidable urgent-care use, patient-reported understanding, and barriers resolved. I would also review documentation quality, escalation patterns, and advocate feedback to identify opportunities to improve the care model.
