# A missed async handoff caused a launch delay

Question details I joined a distributed product team last month, and a task I marked complete before logging off was missing key context for the colleague starting work six hours later. They paused the release until I came online, which delayed the launch.

How should I address this with my manager and improve my handoff notes without making every update unnecessarily long?

Keep it factual: “I marked the task complete without documenting the remaining decision and release dependency. I’ll add a handoff checklist before I log off.” Don’t over-explain or imply the colleague was at fault.

Use a short, consistent handoff structure: status, links, decisions made, open questions, owner, and the next action. Put urgency and blockers first, such as “Blocked: release needs approval of X; if unanswered, hold deployment.” This makes notes scannable rather than long. For work that crosses a time-zone boundary, ask the next person to acknowledge the handoff when the release is time-sensitive. The caveat is that a checklist won’t replace a live overlap for genuinely high-risk launches.
