The role, at a glance.
Lyra Health is hiring a licensed clinical professional to provide remote, longitudinal case management for clients with complex behavioral-health needs. The specialist assesses risk, supports clients during crises, develops care plans, and connects members with appropriate higher levels of care and treatment facilities. The work relies on phone, email, occasional text communication, collaboration with providers and health plans, and independent management of a high-risk caseload. Candidates must hold an independent U.S. clinical license such as LCSW, LMFT, LPC, or LPCC and be U.S. residents. The scheduled shift is Monday through Friday, 9:00 a.m. to 5:30 p.m. Pacific Time, with participation in an on-call rotation.

Sample interview questions
I begin with a calm, direct risk and needs assessment to understand immediacy, intent, plan, means, protective factors, and available supports. I use de-escalation and collaborative safety planning, involve emergency or crisis resources when clinically indicated, and document the assessment and follow-up plan clearly.
I use motivational interviewing to understand the client's goals, ambivalence, practical barriers, and readiness for change. I connect treatment recommendations to what matters to the client, offer realistic options, and maintain consistent follow-up so that engagement can progress at a manageable pace.
I would confirm the client’s clinical needs and preferences, identify suitable in-network or authorized options, and communicate the relevant clinical and logistical information with appropriate consent. I would track referrals, authorization requirements, admission status, and discharge planning while keeping the client informed throughout the process.
I prioritize cases based on acuity, safety risk, treatment deadlines, and follow-up needs, then use structured documentation and task tracking to ensure continuity. I also escalate concerns appropriately, participate actively in multidisciplinary consultation, and maintain boundaries that support timely, reliable client communication.
I use validated measures alongside clinical interviewing rather than as stand-alone decisions. Trends in scores can help identify worsening symptoms, guide care-plan discussions, support level-of-care recommendations, and provide a concrete basis for monitoring response to treatment over time.
