Wheel is seeking a physician leader to design and maintain clinical protocols for virtual urgent, on-demand, and convenience-care programs. The Medical Director will lead clinical quality assurance, use AI-enabled workflows to scale review processes, and convert QA findings into actionable clinician feedback. This role also owns patient-safety escalation processes and outcome-measurement frameworks for continuous care improvement. Candidates need an MD or DO, board certification in a primary-care-relevant specialty, active state licensure, substantial telemedicine experience, and prior clinical leadership experience.

I would define inclusion and exclusion criteria, escalation thresholds, documentation requirements, and outcome measures, then pilot the protocol and refine it using QA findings and patient-outcome data.
I would establish a risk-based review framework that combines representative sampling with targeted reviews of high-risk encounters, safety events, and outlier clinicians. I would use structured rubrics, calibrated reviewers, timely clinician feedback, trend reporting, and automation or AI support where validated, while retaining clinical oversight for consequential decisions.
I would use clear triage rules, red-flag symptoms, exclusion criteria, and escalation pathways that prioritize appropriate in-person or emergency evaluation when needed. I would monitor safety metrics such as repeat visits, escalations, adverse events, and diagnostic misses, then use those findings to improve protocols and clinician education.
I would translate clinical requirements into explicit decision points, workflows, acceptance criteria, and safety guardrails that product and engineering teams can operationalize. I would participate in testing, review edge cases, validate clinical content before release, and monitor post-launch performance to identify needed iterations.
I would assess quality, safety, access, and operational performance together. Core measures would include protocol adherence, escalation rates, repeat utilization, patient outcomes, patient satisfaction, clinician feedback, time to resolution, prescribing patterns where relevant, and variation across cohorts or partners.

About the Role
We’re hiring a Medical Director to own the protocols driving our virtual on-demand, urgent, and convenience care lines. You’ll also lead and scale the QA program that measures their success. It’s fast-moving work inside a practice built with modern, AI-powered tools.
What You’ll Do
Care protocols. Write, update, and refine the clinical protocols behind Wheel’s care programs. Integrate them across all our partners, scale them effectively, and translate them into the logic our platform runs on.
Quality Assurance. Lead Wheel’s clinical QA program. Integrate AI to enable further scale, and turn its findings into feedback the network can act on.
Outcomes and iteration. Develop the frameworks and procedures that track patient and clinical outcomes across these care programs, and use that data to improve care over time.
Patient safety. Manage clinical patient-safety escalation protocols and active escalations, keeping Wheel’s clinical standards consistent and on target.
Who You Are
MD or DO from an accredited institution, board-certified in Family Medicine, Internal Medicine, or Emergency Medicine. Primary care experience preferred.
Substantial clinical practice experience, including virtual care or telemedicine.
Prior clinical leadership experience managing teams directly in telehealth, digital health, or a high-volume virtual practice.
Hands-on experience with clinical quality assurance — building, running, or auditing a QA program.
Experience authoring and maintaining clinical protocols.
Basic proficiency with AI tools & software

Salary and Perks
Pay Range: $250,000 – $290,000 plus a 15% bonus.
Final offer amounts are determined by multiple factors including, but not limited to, the scope and responsibilities of the role, the selected candidate’s work experience, education and training, the work location as well as market and business considerations.
