Sample interview questions
Describe how you have applied High Reliability Organization principles in a healthcare setting.

I began by assessing current safety culture, leadership behaviors, and reporting practices, then partnered with clinical and operational leaders to prioritize interventions. I used HRO principles such as sensitivity to operations, deference to expertise, and preoccupation with failure to build practical routines for escalation, learning, and follow-through. I tracked leading and lagging measures to demonstrate progress and adjusted the approach with frontline feedback.

How would you turn safety and behavioral observation data into recommendations for a client executive team?

I would first validate the data, define the operational question, and segment findings by relevant care setting, role, and trend. I would combine quantitative results with qualitative context from staff interviews or observations to identify likely root causes. For executives, I would present a concise narrative linking findings to patient and workforce risk, prioritize actions by impact and feasibility, and recommend measurable next steps.

Tell us about a time you facilitated a difficult safety conversation with frontline staff and leaders.

I would establish psychological safety by clarifying that the discussion is focused on learning and system improvement rather than blame. I would use a structured review of the event, invite perspectives from those closest to the work, and distinguish human error, at-risk behavior, and reckless behavior using just-culture principles. The outcome would be a shared action plan with clear owners, timelines, and feedback loops.

How do you manage a consulting workstream that has tight deadlines, multiple stakeholders, and changing client priorities?

I set a clear workplan with scope, milestones, dependencies, risks, and decision owners at the start of the engagement. I maintain regular client check-ins, communicate emerging risks early, and document scope changes so priorities can be reset transparently. This approach keeps deliverables on track while preserving trust and allowing the team to respond to new information.

What is your approach to delivering training that creates sustained behavior change rather than one-time participation?

I tailor training to the audience's real workflows and use case-based, interactive exercises that let participants practice the desired behaviors. I reinforce learning through leader coaching, job aids, observation, and follow-up measures rather than treating attendance as success. I also gather participant feedback and performance data to refine the program and sustain adoption over time.
