Sample interview questions
How do you ensure accurate code assignment when documentation is incomplete or unclear?

I first review the complete encounter record, including provider notes, orders, operative reports, and ancillary documentation. If the record still does not support a definitive code, I follow established query procedures, document the issue appropriately, and avoid making unsupported assumptions.

Describe your experience with facility E/M leveling in the emergency department.

I review the facility resources documented for the encounter, such as nursing interventions, diagnostics, medications, procedures, and monitoring. I apply the organization’s approved leveling methodology consistently and validate that the assigned level is supported by the full medical record.

What steps do you take to resolve coding or claim edits?

I identify the edit type and review the supporting documentation, coding guidelines, payer rules, and applicable billing requirements. I correct coding when warranted, escalate documentation issues through the proper channel, and track recurring edit patterns to help prevent future denials.

How do you maintain quality while meeting productivity requirements in a remote coding role?

I use a structured workflow that prioritizes work queues by turnaround needs and complexity, while building in time for self-audits on high-risk cases. I monitor my productivity and accuracy regularly, minimize distractions, and promptly ask for clarification when policy or documentation questions arise.

How have you maintained your coding credentials and kept current with regulatory changes?

I maintain a continuing-education plan throughout the credential cycle, attend coding education sessions, and review updates from AHIMA, AAPC, CMS, and payer guidance. I also apply new guidance through practice cases and internal feedback to ensure it is reflected in my daily coding decisions.
