Sample interview questions
I use a structured audit tool to assess clinical validation, identification of documentation opportunities, query appropriateness, coding alignment, and adherence to organizational policy. I document objective evidence from the record, score consistently against defined criteria, and provide feedback that distinguishes education opportunities from material compliance concerns.
I would review the full clinical picture, including provider documentation, diagnostics, treatment, and relevant coding guidelines, to determine whether the condition is clinically supported and reportable. I would then explain the missed opportunity with record-based rationale, applicable guidance, and a practical recommendation for future case review or compliant query consideration.
I keep feedback specific, evidence-based, and tied to established policy or coding and query standards. I invite the specialist to explain their clinical reasoning, review the case together when useful, and focus the conversation on consistent future practice rather than assigning blame.
I would monitor query volume and response rates, agreement rates, audit accuracy, CC/MCC capture, case-mix-index impact, and recurring audit themes. I would use trends to target education, refine workflow guidance, and measure whether corrective actions improve both quality and consistency over time.
I access only the minimum necessary information through approved systems, follow secure authentication and storage requirements, and avoid sharing PHI through unapproved channels. I also maintain a private work environment, apply company retention procedures, and promptly report any suspected privacy or security issue.
