About this role.
About Solace
Healthcare in the U.S. is fundamentally broken. The system is so complex that 88% of U.S. adults do not have the health literacy necessary to navigate it without help. Solace cuts through the red tape of healthcare by pairing patients with expert advocates and giving them the tools to make better decisions—and get better outcomes.
We’re a Series C startup, founded in 2022 and backed by Inspired Capital, Craft Ventures, Torch Capital, Menlo Ventures, Signalfire, and IVP. Our U.S. based team is lean, mission-driven, and growing quickly.
Solace isn’t a place to coast. We’re here to redefine healthcare—and that demands urgency, precision, and heart. If you’re looking to stretch yourself, sharpen your edge, and do the best work of your life alongside a team that cares deeply, you’re in the right place. We’re intense, and we like it that way.

Where You Come In:
Solace is looking for licensed LPNs and LVNs to step into a Patient Advocate role built around Medicare members. Rather than bedside or facility work, you’ll support patients remotely across the full arc of their care. Untangling complicated health situations, moving them toward their wellness goals, and knocking down the Social Determinants of Health (SDOH) that get in their way. The job rewards people who listen closely and then actually do something about what they hear.
Worth flagging up front: This is a 1099 Contract position, benefits are not included. It is simply a flat hourly rate. You control your calendar. Full-time, part-time, or somewhere in the middle. The role is entirely remote. You’ll connect with patients by video or phone, never in person.
Your Day-to-Day
Lean on your LPN/LVN clinical background while getting fluent in Solace’s proprietary platform, tools, and standards.
Turn empathy and attentive listening into patient relationships built on genuine trust.
Read each patient’s situation, sort out what matters most first, and shape a care plan that keeps things moving over time.
Design care plans that go beyond the clinical, factoring in everyday barriers like access to food, getting to appointments, and support in the home.
Who We’re Looking For
A current LPN or LVN license in good standing.
A track record of working with patients across many different backgrounds and communities.
Compassion: your LPN/LVN experience is rooted in caring about people and standing up for those who can’t stand up for themselves.
Steadiness and structure: you keep multiple priorities organized and hold it together when the pace picks up.
Quick technical footing: unfamiliar systems don’t intimidate you — you get comfortable fast.
Ownership: you spot what needs handling and handle it, without waiting for a nudge.
Applicants must be based in the United States.
Up for the Challenge?
We look forward to meeting you.

Sample interview questions
I would begin by listening carefully to understand the patient's immediate concerns, health goals, and barriers. I would assess urgency, prioritize safety-critical needs first, and build a clear care plan with manageable next steps. I would also identify resources for issues such as transportation, food access, or home support and follow up to confirm progress.
I use active listening, plain language, and a calm, respectful tone. I confirm my understanding of the patient's concerns, explain options without jargon, and make realistic commitments about next steps. Consistent follow-up is essential because it shows patients that they are being heard and supported.
I use structured task prioritization, detailed documentation, and reminders for follow-ups. I categorize work by clinical urgency, time sensitivity, and barriers that could delay care. At the end of each interaction, I document actions, ownership, and the next contact point so that plans remain clear and continuous.
If a patient repeatedly misses appointments because they lack transportation, I would first confirm the specific limitation and then help identify covered transportation benefits, local services, or telehealth alternatives where appropriate. I would coordinate with the relevant provider or resource and verify that the patient understands how to use the solution.
I learn new systems by first understanding the required workflow and patient-safety implications, then practicing core tasks and using available reference materials. I ask focused questions early, document process notes, and seek feedback to ensure my work is accurate. My goal is to become efficient without sacrificing patient privacy or quality.
