Sample interview questions
How do you prepare for and conduct a routine monitoring visit?

I review the monitoring plan, prior visit reports, outstanding action items, recruitment progress, safety information, and EDC data before the visit. On site, I verify informed consent, source data, investigational product accountability where applicable, essential documents, protocol adherence, and staff training. I discuss findings with the site, agree on realistic corrective actions, and submit a timely, accurate monitoring report.

Describe how you would manage a recurring protocol deviation at an investigative site.

I would first confirm the deviation details and assess patient safety, data integrity, and regulatory impact. I would discuss root causes with the investigator and site staff, document the issue per the protocol and SOPs, and implement a CAPA plan that may include retraining, workflow changes, and enhanced follow-up. I would escalate material issues to the project team and monitor the site until sustained compliance is demonstrated.

What steps do you take to ensure source-data quality and EDC accuracy?

I use a risk-based approach to compare critical EDC entries with contemporaneous source records, focusing on eligibility, endpoints, safety data, dosing, and visit dates. I raise clear queries, verify their resolution, and check for consistency across source, EDC, laboratory, and other trial systems. I also identify trends early and provide targeted site guidance to prevent repeated errors.

How would you support an underperforming site that is missing recruitment targets?

I would assess the site's recruitment funnel, competing studies, staffing capacity, patient population, and feasibility assumptions. Working with the investigator and study team, I would develop a practical recovery plan, such as refining pre-screening processes, retraining staff on eligibility criteria, increasing outreach within approved methods, and setting measurable follow-up milestones. If performance remains inadequate, I would communicate the risk promptly and support escalation or mitigation planning.

How do you mentor a junior CRA while maintaining quality and accountability?

I set clear expectations, model effective monitoring preparation and documentation, and use co-monitoring visits to provide specific feedback. I encourage the CRA to explain their rationale for findings and follow-up actions, then coach on risk assessment, site communication, and escalation pathways. I document development goals and progressively increase independence while ensuring that patient safety, compliance, and data quality remain protected.
