Sample interview questions
How do you prepare for and conduct a monitoring visit at an investigative site?

I review the monitoring plan, prior visit reports, open action items, enrolment status, safety information, data queries, and essential documents before the visit. During the visit, I verify informed consent, source data, investigational product accountability, protocol adherence, and regulatory documentation, then document findings and agree clear corrective actions with the site.

Describe how you would manage a significant protocol deviation discovered during source-data review.

I would first confirm the facts against the protocol, source records, and applicable procedures, then promptly notify the appropriate site and study stakeholders. I would ensure the deviation is documented, assess its potential impact on participant safety and data integrity, support required reporting, and follow up on a corrective and preventive action plan.

How do you help an underperforming site improve recruitment while maintaining compliance?

I assess the root causes, such as patient identification processes, staffing, competing studies, or protocol understanding, and discuss practical compliant solutions with the site. I reinforce eligibility requirements, provide targeted training, establish measurable actions and timelines, and monitor progress closely while escalating risks early to the project team.

What steps do you take to maintain TMF and ISF inspection readiness?

I reconcile essential documents regularly, verify that records are complete, current, accurate, and filed within required timelines, and resolve discrepancies promptly. I also ensure monitoring reports, follow-up communications, training records, and site documents provide a clear and contemporaneous audit trail.

How would you mentor a CRA who is new to independent monitoring?

I would set clear expectations, review the monitoring plan and visit preparation process with them, and use co-monitoring to provide structured observation and feedback. I would focus on risk-based prioritisation, communication with sites, documentation quality, and confidence in escalating issues, with follow-up development goals after each visit.
