Core skills

Skills and capabilities most closely associated with this opportunity.

Sample interview questions
How do you approach an initial in-home assessment for a medically complex older adult?

I begin by reviewing available records and recent utilization, then assess medical conditions, medications, functional status, cognition, behavioral health, caregiver support, and home safety. I prioritize immediate risks, align the care plan with the patient’s goals, and ensure the next clinical and coordination steps are clearly documented.

Describe how you would reduce avoidable readmissions after a patient is discharged from the hospital.

I would arrange timely post-discharge outreach or a home visit, reconcile medications, verify that follow-up care and services are in place, and identify barriers such as mobility, food access, or caregiver capacity. I would establish clear escalation instructions and collaborate with the broader care team to address emerging symptoms before they require emergency care.

How do you balance independent decision-making in the field with interdisciplinary collaboration?

I make timely clinical decisions within my scope using the patient’s presentation, history, and available data, while proactively involving colleagues when their expertise improves safety or follow-through. Clear documentation and concise communication help the entire team understand the plan, responsibilities, and warning signs.

What strategies do you use to build trust with patients and caregivers in their homes?

I listen carefully, acknowledge the realities of the patient’s environment, and explain recommendations in plain language. I involve caregivers with the patient’s consent, set realistic shared goals, and follow through consistently so patients see that the care plan is responsive to their needs.

How would you use clinical technology to improve care for a high-risk patient panel?

I would use clinical decision-support tools to identify care gaps, high-risk trends, medication issues, and preventive opportunities. Technology should inform—not replace—clinical judgment, helping me prioritize outreach and deliver proactive, evidence-based interventions.

Opportunity details

About this role.

Practice medicine where it can matter most: in the home.

Clover Health is looking for exceptional, mission-driven physicians to join our field-based In-Home Care team. In this role, you will care for Medicare Advantage members with complex medical, functional, and social needs — many of whom are older, medically fragile, and at high risk for avoidable complications.

This is not transactional care. This is relationship-centered, outcomes-driven medicine.

As a Field-Based Staff Physician, you will be accountable for the health outcomes of a defined panel of members. You will meet patients where they are, understand the realities of their home environment, identify risks early, and help coordinate the right care at the right time. You will have the autonomy to practice thoughtfully, the support of an interdisciplinary team, and the technology to act with greater insight and precision.

We are seeking physicians who are highly accountable, clinically strong, operationally effective, and deeply committed to doing what is best for vulnerable seniors.

Why this role matters

Many of the patients we serve are navigating multiple chronic conditions, recent hospitalizations, fragmented care, limited resources, and the challenges that come with aging. For these members, the home is often where the most important clinical information lives.

In this role, you will have the opportunity to see the full picture: the medications on the kitchen table, the caregiver dynamics, the fall risks, the barriers to follow-up, and the early warning signs that may never surface in a clinic visit.

Your work will directly influence whether members recover safely, remain stable, avoid unnecessary hospitalizations, close care gaps, and feel supported by a care team that genuinely knows them.

What you’ll do

As a Field-Based Staff Physician on Clover’s In-Home Care team, you will:

  • Provide high-quality, patient-facing care in members’ homes across New Jersey
  • Manage a panel of medically complex Medicare Advantage members and take full accountability for clinical outcomes
  • Conduct comprehensive in-home assessments that evaluate medical, functional, behavioral, and social needs
  • Lead post-discharge and transitional care visits to support safe recovery and reduce avoidable readmissions
  • Identify changes in clinical status early and intervene proactively
  • Manage chronic conditions, medication concerns, care gaps, and acute clinical needs within the home-based setting
  • Build trusted relationships with members, caregivers, and families
  • Collaborate closely with nurses, care managers, pharmacists, advanced practice clinicians, primary care providers, specialists, and community partners
  • Use Clover Assistant and other clinical technology tools to surface risks, guide evidence-based decisions, and deliver more proactive care
  • Communicate clearly and efficiently with internal and external care teams
  • Document accurately and thoughtfully to support continuity, quality, and accountability
  • Help advance a care model built around value, outcomes, and human connection

You’ll thrive here if you are

  • Mission-driven and energized by caring for older adults with complex needs
  • Highly accountable and willing to own the outcomes of your patient panel
  • Comfortable practicing independently in the field while staying closely connected to a team
  • Clinically thoughtful, decisive, and able to prioritize what matters most
  • Effective and efficient in how you manage your time, visits, follow-up, and documentation
  • Tech-savvy and curious about how data and clinical tools can improve care
  • A strong communicator who builds trust with patients, caregivers, and colleagues
  • Motivated by value-based care and the opportunity to improve outcomes, not simply complete visits
  • Compassionate, practical, and deeply committed to doing what is right for sick and underserved seniors

What you bring

  • MD or DO degree
  • Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field
  • Geriatrics experience or fellowship training preferred
  • Active New Jersey medical license, DEA registration, and CDS registration
  • Strong clinical judgment and experience managing chronic conditions in older adults
  • Comfort caring for medically complex, frail, and socially vulnerable patients
  • Ability to work independently in a field-based environment
  • Strong documentation, communication, and care coordination skills
  • Curiosity and proficiency with clinical technology, including the ability to use tools such as Clover Assistant to make more informed decisions and identify care opportunities
  • Experience in home-based care, value-based care, population health, complex care, or Medicare Advantage is a plus
