What does an extra interview after the final round mean?

Question details

I completed what the recruiter called the final interview and was told they would decide this week. Instead, they asked me to meet a senior manager who was not part of the original process, without explaining the purpose. I am mildly concerned the role or their confidence in me has changed.

Is this usually an approval step or a sign of hesitation?
What should I ask the recruiter before the meeting?
How should I prepare if the manager repeats earlier questions?
