Sample interview questions
I begin by confirming the patient’s identity, location, privacy, emergency contact information, and ability to participate safely by video. I then complete a structured psychiatric, medical, medication, substance-use, and psychosocial assessment, including a risk evaluation, before documenting findings and collaboratively developing a treatment plan.
I use direct, evidence-based risk questions to assess ideation, intent, plan, means, protective factors, and level of acuity. If risk is elevated, I follow an immediate safety protocol that may include a collaborative safety plan, contacting emergency supports, coordinating with local crisis resources, and documenting all actions clearly.
I confirm that telehealth prescribing is clinically appropriate and legally permitted in the patient’s state, review diagnosis and contraindications, reconcile medications, and discuss benefits, risks, alternatives, and follow-up expectations. I also consider the patient’s history, monitoring needs, access to pharmacy services, and coordination with other providers.
With appropriate consent, I would communicate relevant diagnostic impressions, medication plans, safety concerns, and treatment goals while respecting role boundaries and privacy requirements. I would use concise, timely updates and seek therapist observations that may improve medication management and continuity of care.
I create a professional, private, and empathetic virtual environment; use clear language; verify patient understanding; and set reliable follow-up plans. I also monitor outcomes, medication adherence, side effects, missed appointments, and patient feedback so that treatment can be adjusted promptly.
UpLift – Redefining Access to Mental Healthcare
At UpLift, we believe mental health is just as important as physical health — and we’re rethinking the mental healthcare model. UpLift is a multi-sided marketplace, applying technology to empower patients to find Therapists and Psychiatric providers covered by their insurance plan.
While 1 in 5 Americans struggle with mental health issues, only 40% of this group received care in the last year. At the same time, only 30% of therapists accept insurance. UpLift acts as the bridge between patient, provider, and insurer — empowering patients to find the best care for their needs at a price they can afford.
