Sample interview questions
I establish a consistent agenda that includes caseload review, clinical formulation, risk assessment, documentation feedback, professional development goals, and space for the clinician’s questions. I tailor the depth of guidance to each therapist’s experience while documenting supervision clearly and following up on agreed actions.
I would first assess immediate safety concerns, including risk factors, protective factors, current supports, and whether emergency intervention is needed. I would guide the clinician through an appropriate safety plan, documentation, consultation, and escalation process while ensuring actions align with agency policy and applicable regulations.
I stay current on applicable regulations, translate requirements into practical workflows, and routinely review documentation and clinical processes for gaps. I also use supervision and training to reinforce minimum-necessary information sharing, secure recordkeeping, timely notes, and consistent treatment planning.
I use protected time blocks for supervision, documentation review, administrative tasks, and direct care, prioritizing safety-sensitive and time-sensitive matters first. Clear expectations, organized tracking systems, and proactive communication with leadership help ensure both clinicians and clients receive timely support.
I would address concerns promptly using specific observations, relevant documentation standards, and clearly defined expectations. I would collaborate on an improvement plan with measurable goals, training or consultation support, regular check-ins, and transparent escalation steps if improvement does not occur.
