Sample interview questions
How would you design a clinical QA framework for a new AI feature that supports therapy clients?

I would begin by defining the feature's intended use, user groups, clinical boundaries, and foreseeable failure modes. I would then create risk-based evaluation rubrics covering safety, accuracy, bias, crisis handling, privacy, and alignment with therapeutic standards, with clear pass/fail thresholds and escalation paths. Finally, I would implement pre-release testing, post-release monitoring, structured issue tracking, and recurring reviews with clinical, product, legal, and compliance stakeholders.

Describe how you would assess an AI response that may present a clinical safety concern.

I would first classify the concern by severity, immediacy of potential harm, affected population, and whether the output violates defined clinical or policy standards. I would preserve the relevant context, document the failure mode, determine whether immediate mitigation is needed, and route the issue through the established clinical-risk escalation process. I would also identify whether the root cause points to a prompt, guardrail, workflow, model behavior, or user-experience issue and recommend corrective actions.

How would you balance rigorous clinical review with the speed needed in an AI product environment?

I would use a tiered, risk-based approach so high-impact clinical and crisis-related use cases receive the most intensive review, while lower-risk changes follow streamlined but documented checks. Clear rubrics, reusable test sets, sampling plans, and severity definitions help reviewers make consistent decisions quickly. I would also use trend reporting to focus effort on the highest-frequency and highest-severity failure patterns.

How do you communicate clinical QA findings to engineering and product teams?

I translate findings into concrete examples, measurable impact, severity, and prioritized recommendations rather than relying only on clinical terminology. For each issue, I would explain the user risk, the applicable quality standard, likely contributing factors, and an actionable proposed change to prompts, safeguards, workflows, or monitoring. I would confirm shared understanding with stakeholders and track the recommendation through validation after implementation.

What metrics would you use to monitor the quality of an AI tool used in mental health?

I would monitor clinical-safety incident rates, severity distribution, escalation volume, evaluation-rubric pass rates, accuracy and appropriateness scores, bias indicators, and time to resolution for identified issues. I would segment these measures by use case and risk category to detect concentrated problems. Qualitative feedback from clinicians, clients, and support teams would complement quantitative monitoring and help identify emerging failure modes.
