Sample interview questions
I begin by assessing immediate safety and clinical risks, then clarify the patient's goals and barriers to following the care plan. I document a prioritized plan and coordinate with advocates on practical needs such as transportation, food access, medication access, and home support, while defining clear follow-up steps.
I use plain language, active listening, and teach-back to ensure the patient understands the plan. I allow time for questions, acknowledge concerns respectfully, and involve caregivers or advocates with patient consent so that the patient has reliable support after the visit.
I would establish clear roles: I am responsible for clinical assessment, medical decisions, and escalation, while the advocate helps remove logistical and social barriers. I would communicate concise, actionable care-plan elements and maintain timely documentation so the entire team works from the same information.
In a previous transition, I learned the new workflow through focused practice, identified common documentation and handoff risks, and created a consistent personal checklist. I shared practical feedback with the team and adjusted my process quickly while protecting patient safety and continuity of care.
I would monitor patient access, follow-up completion, medication and referral adherence, avoidable urgent-care use, patient-reported understanding, and barriers resolved. I would also review documentation quality, escalation patterns, and advocate feedback to identify opportunities to improve the care model.
