Sample interview questions
How would you prioritize needs for a Medicare member facing several medical and social barriers?

I would first assess immediate safety and urgent clinical concerns, then identify barriers that could prevent the patient from following the care plan. I would collaborate with the patient to set realistic priorities, document clear next steps, and follow up consistently to ensure progress.

Describe how you build trust with a patient during a remote phone or video interaction.

I start by listening without interruption, using plain language, and confirming my understanding of the patient’s concerns. I explain what I can do, set clear expectations, and follow through on commitments so the patient sees that their needs are being taken seriously.

How would you support a patient who repeatedly misses appointments because of transportation challenges?

I would explore the specific transportation barrier, review available benefits and local resources, and coordinate with the patient and care team on workable alternatives. This could include arranging eligible transportation services, considering telehealth where appropriate, and confirming the plan before the next appointment.

What steps do you take to stay organized while managing multiple patient cases independently?

I use structured task lists, documented care plans, reminders, and regular case reviews to track commitments and follow-ups. I prioritize by urgency and patient impact, communicate early when a barrier arises, and make sure each interaction results in a clear next action.

How do you adapt to unfamiliar healthcare platforms or documentation tools?

I learn the workflow by using training materials, practicing common tasks, and asking focused questions when needed. I prioritize accurate and timely documentation because it supports continuity of care, informed decisions, and accountability across the patient journey.

Where You Come In:
Solace is looking for licensed LPNs and LVNs to step into a Patient Advocate role built around Medicare members. Rather than bedside or facility work, you’ll support patients remotely across the full arc of their care. Untangling complicated health situations, moving them toward their wellness goals, and knocking down the Social Determinants of Health (SDOH) that get in their way. The job rewards people who listen closely and then actually do something about what they hear.

Worth flagging up front: this is a 1099 independent contractor engagement. You control your calendar. Full-time, part-time, or somewhere in the middle. The role is entirely remote. You’ll connect with patients by video or phone, never in person.

Your Day-to-Day

  • Lean on your LPN/LVN clinical background while getting fluent in Solace’s proprietary platform, tools, and standards.

  • Turn empathy and attentive listening into patient relationships built on genuine trust.

  • Read each patient’s situation, sort out what matters most first, and shape a care plan that keeps things moving over time.

  • Design care plans that go beyond the clinical, factoring in everyday barriers like access to food, getting to appointments, and support in the home.

Who We’re Looking For

  • A current LPN or LVN license in good standing.

  • A track record of working with patients across many different backgrounds and communities.

  • Compassion: your LPN/LVN experience is rooted in caring about people and standing up for those who can’t stand up for themselves.

  • Steadiness and structure: you keep multiple priorities organized and hold it together when the pace picks up.

  • Quick technical footing: unfamiliar systems don’t intimidate you — you get comfortable fast.

  • Ownership: you spot what needs handling and handle it, without waiting for a nudge.

Applicants must be based in the United States.

Up for the Challenge?

We look forward to meeting you.
