Sample interview questions
I begin by establishing psychological safety and clarifying each person’s individual and shared goals. I use collaborative assessment and transparent treatment planning to identify workable areas of agreement while respecting differences, monitoring progress and revisiting goals as needed.
I prioritize safety, trust, choice, collaboration, and empowerment. I assess how trauma may influence communication, attachment, emotional regulation, and conflict, then pace interventions carefully to avoid re-traumatization while building coping skills and relational stability.
I conduct an immediate, structured risk assessment, address urgent safety needs, and follow organizational and legal reporting protocols. I document objectively, consult with supervision or the treatment team when appropriate, create a safety plan, and arrange a higher level of care or emergency support when indicated.
I complete notes promptly, use objective and clinically relevant language, document interventions and client response, and ensure records support the treatment plan. I follow HIPAA, agency procedures, payer requirements, and applicable New York State regulations when using the EHR.
I would lead with cultural humility, curiosity, and respect for the client’s lived experience. I would explain confidentiality and the therapy process clearly, validate concerns about care systems, identify practical barriers, and collaboratively develop goals that feel meaningful and accessible to the client.
